Event background:

The Parliamentary Review of Health and Social Care 2018 highlighted the compelling case for change in the delivery of health and social care in Wales. Demographic and social changes coupled with increasing prevalence in the burden of chronic diseases require new approaches in the planning and provision of care services to patients, communities and society in the future.

In order to initiate such developments, there needs to be greater collaboration and partnership working across agencies, with wider involvement from patients and the public to inform the nature and type of services provided.

It is in anticipation of such a changing landscape, the HWA has been delivering a range of innovative projects, involving staff, students and partners that are having a significant impact on the wellbeing of communities.

Delegates will hear of the exciting translational research facilitated through the Academy and be made aware of the innovative approaches to the delivery of health and social care emerging from the collaborative ventures within the Academy.

Tours of the HWA will be available and staff and students will be on hand to demonstrate how the research-led services can support the wider system of health and social care delivery.

Roedd newidiadau demograffig a chymdeithasol ynghyd â chyffredinrwydd cynyddol yn y baich o glefydau cronig yn gofyn am ddulliau newydd yn y cynllunio a darparu gwasanaethau gofal i gleifion, cymunedau achymdeithas yn y dyfodol.

Er mwyn cychwyn datblygiadau o’r fath, mae angen bod yn fwy cydweithredol a gweithio mewn partneriaeth ar draws asiantaethau, gyda chyfranogiad ehangach gan gleifion a’r cyhoedd i hysbysu natur a math y gwasanaethau a ddarperir.

Mae’n rhagweld y bydd tirlun mor newidiol, mae’r HWA wedi bod yn darparu amrywiaeth o brosiectau arloesol, yn cynnwys staff,myfyrwyr a phartneriaid sy’n cael effaith ar les cymunedau.

Bydd cynrychiolwyr yn clywed am yr ymchwil trawsfudol cyffrous wedi’i hwyluso trwy’r Academi a chael bod yn ymwybodol o’r dulliau gweithredu arloesol tuag at ddarparu iechyd a gofal cymdeithasol sy’n dod i’r amlwg o’r mentrau cydweithredol o fewn yr Academi.

Bydd teithiau’r HWA ar gael a bydd staff myfyrwyr wrth law i ddangos sut y gall y gwasanaethau a arweinir gan ymchwil gefnogi’r system ehangach o ddarparu iechyd a gofal cymdeithasol.
